BRIEF DESCRIPTION: The Unit sees patients who are medically stable but have ongoing exacerbation of their chronic disease. Typical conditions; infective exacerbation of COPD, Cellulitis, Heart Failure, Parkinson Disease and Diabetes
Patients are either discharged from acute care and waiting to go home or they have been referred from the Community in order to stop their illness becoming acute (admission avoidance). The Unit also provides palliative care,
WHAT YOU MIGHT SEE/DO AT THE UNIT: Every day at the Unit is different, there is a lot of variety, but you can expect to see patient reviews, assessment of patients’ mental status, multi-disciplinary working (eg. speech therapy, physiotherapy). The unit also sees frail elderly patients who have had orthopaedic surgical interventions after a fracture and then come to the unit for assessment before they are able to go home, this might include investigating why they had a fall in the 1st place, and identifying needs to allow discharge, which may mean liaising with staff in the community
There will be opportunities to take histories and improve your examination skills (heart sounds, neuro examinations) and perform a variety of clinical skills, for example, Blood Pressure, IV and cannulation, ECGs, Catheterisation.
