Option 1 - (off site) Park Rehabilitation Centre, Badsley Moor Lane, Rotherham, S65 2QL Option 2 - Stroke Rehabilitation Team, Therapy Services, C Level, Junction 3, Rotherham Hospital BRIEF DESCRIPTION: The Stroke Rehabilitation Team delivers patient centered care throughout the stroke pathway from acute admission, through discharge and up to 6 months post-discharge, with the overall goal to improve the quality of the patient journey. This is a unique and innovative single pathway approach to the delivery of stroke care, that provides a seamless transfer of care without delays for patients. The team are led by individual patient need to tailor the intensity and nature of assessment and therapy input for each patient. We provide Physiotherapy, Occupational Therapy, Speech Therapy and Clinical Psychology to patients through this pathway WHAT YOU MIGHT SEE/DO AT THE CLINIC: Option 1: Fridays - shadow a 6 month clinic which is held at the Park Rehabilitation Centre on a Friday morning 09:00-12:00, by our Band 4 Assistant Practitioner. This clinic is a health and social care review for all Stroke patients at a 6 month point so would be a good overall experience for the students - 2 students per session Option 2: Tuesday - Shadow sessions with one of our Speech Therapists (Lawrence Mellor) for a morning 08:00-12:00 1 student per session KEY CONTACTS: Alison Mallett, Senior Physiotherapist Tel: 01709 427315 Email: [email protected] |
